How To Obtain a Certificate of Attendance

* The Collins Hill Community School office issues Certificate of Attendance.

* Hours are Monday thru Thursday 1:30 p.m. to 6:30 p.m.

* During school hours the student can obtain the form outside the door of the office, fill it out-place the form and the $3.00 fee required in the envelope provided – then place the sealed envelope in the metal security box outside the office door.

* Processing time is 48 hours.

* The student must come back to the office after 48 hours to ask the office staff for their completed form.

* The Certificate of Attendance is good for 30 days, then it expires.

* Certificate of Attendance is required for a GA Driver’s permit and GA Driver’s license.
