Registration Requirements:
When registering your child at W.J. Cooper Elementary,
you will need to provide the following:

The following documents are required for school enrollment:

• An official document showing proof of birth date. (Required for kindergarten and first-grade registration only.) Examples include an original birth certificate or birth registration, or an official state or federal form with date of birth.

• Proof of residency in attendance zone. Examples include a non-contingent sales contract, lease, or deed. A lease or deed must be accompanied by at least one utility bill (excluding telephone bills) with the same address. A contingency contract is not acceptable. Call your local school if you live with a friend or family member and need information about verifying your residency.

• Evidence of vision, hearing, and dental exams obtained in the past year. This information must be on Georgia Form #3300, available from the health department or your physician and dentist. Letters from appropriate healthcare professionals and out-of-state certificates are acceptable, if completed within the last 12 months and stapled to the state form.

• A Georgia certificate of immunization. State form #3231(revised March 2007) must be completed by the health department or your private physician. Required shots include the series for Hepatitis B, DPT, and Polio; an MMR shot, plus additional mumps and measles shots or a second MMR, or healthcare provider documentation of illness; and 2 doses of the varicella (Chicken Pox) vaccine or documentation of illness. A valid Form #3231 must be marked in the box with either “Complete for School Attendance” or “Date of Expiration.” By state law, a valid immunization form or immunization exemption (for medical or religious reasons) is required for enrollment. You may request an exemption from your local school.
• Proof of custody/guardianship. A Letter of Guardianship is required for registration if the enrolling adult is not the birth parent. To learn more, contact Gwinnett County Probate Court at 770-822-8265.

• The child’s social security number. State law requires that schools ask for this number at the time of enrollment. Students will be enrolled, even if the number is not provided. However, parents will be asked to sign a waiver.

• A photo ID. The school may ask the enrolling adult to present a photo ID.



Normal enrollment hours during the school year are from 8:30 - 10:30 Monday through Friday.

Early Pick Up of Students
If it becomes necessary to pick up students before the end of the school day, call for them at the office, not the classroom. The child will be paged to come to the office while the parent signs the child out on the card in the office. Be prepared to show picture identification such as a driver’s license. Children cannot be released to adults other than parents without permission of the parents. Parents should not make it a daily occurrence to come into the school to check out their child at the end of the day to avoid the car rider pick-up lane. No child will be checked out in the front office between 3:00 and 3:30 P.M. due to the staff’s involvement in afternoon bus call and other office responsibilities.

School Closings
Sometimes unusual weather may cause the school to be closed. School closings are determined by the Gwinnett County School Administrative Offices and not by Cooper. Announcements of closings will be broadcast via local news media. The basic premise to observe is that the school IS open unless it is specifically announced otherwise. The official radio station for the school closing announcements is WSB AM 750. Please do not call Cooper Elementary as the school needs to be in touch with the Administrative Offices regarding emergency announcements.

Parent Involvement Opportunities
Cooper needs active, involved parents. Below are some committees in which parents may wish to become involved. Contact a PTA officer or the school if you would like to help in any of these areas:

Volunteers
Volunteers can become involved with tutoring, helping in the computer labs, helping in the classrooms, helping in the media center, and various other activities. Please contact your child’s teacher for information.

Room Mothers
This committee assists our room mothers in the various duties they have throughout the year. Room mothers help coordinate 2 parties during the year and assist the teacher in recruiting volunteers for various activities. We are asking for each class to have two room moms/dads this year.

Media Committee
These committees help plan and select materials for instructional use. Parents are urged to volunteer their time and expertise. Contact the media specialist if you are interested.

Field Trip Chaperones
Parents will be asked to act as chaperones on field trips the students take during the school year. Unfortunately, our policy must be that younger siblings may not accompany parents on the school bus or on the field trip. We appreciate your cooperation. All students going on a field trip must ride the school bus with the other students in order to be eligible to go on the field trip.

School Council
The School Council is a seven member elected committee, which was a mandate of HB 1187, the Educational Reform Act of 2000. The Council serves as an advisory board to the school under established guidelines set forth by the state legislature. The parent and teacher representatives on the Council will be elected in September, and serve a two year term.
