    Non-Tobacco Users Information for State Health Benefit Plan Participants
    State Health Benefit Plan Policy for Non-Tobacco Users
    The State Health Benefit Plan (SHBP) has implemented a policy for non-tobacco users who wish to have the tobacco surcharge removed. Members can have their tobacco surcharge removed by completing a wellness requirement. They must answer a Health Assessment Questionnaire and access wellness information on their health plan’s web site.
    Specific instructions for accessing more information are included at the web site
    www.dch.georgia.gov/shbp_plans. Members who have not used tobacco products in the past 12 months and complete this program will have the tobacco surcharge removed from future premium payments for this Plan Year.
    Upon completion of the wellness requirement, obtain the
    Non-Tobacco User’s Affidavit Form available online at www.dch.georgia.gov/shbp_plans.
    Once, you have read and signed the affidavit, you must submit it to the GCPS Benefit department for submission to the SHBP.
