CIGNA Healthcare
are the two State Health Plans offered to GCPS employees. The coverage program and premiums are the same for both plans. Review the
Employee State Health Decision Guide
for employees whose benefits became effective on 1/1/12 or
New Employee State Health Decision Guide
for employees whose benefits became effective
after
1/1/12.
Be sure to check with plan your personal healthcare provider(s) participate.
State Health also offers a
Tricare Supplement
for employees who are retired from the military, a spouse of a retired military or active duty service member, or enrolled in TRICARE Reserve Select (TRS), or a spouse of a disable veteran and have CHAMPVA.

Peachcare for Kids
. Coverage may begin as soon as January 1, 2012.

A
spousal surcharge of $50
will be applied to monthly premium if employee's spouse has access to coverage through their employer.
A
tobacco surcharge of $80
will be applied to monthly premium if employee or dependent(s) are tobacco users.

State Health Tobacco Surcharge Information
The State Health Benefit Plan (SHBP) Policy has implemented a policy for non-tobacco users who wish to have the tobacco surcharge removed. Members can have their tobacco surcharge removed by completing a Health Assessment Questionnaire.

for specific instructions for accessing more information. Members who have not used tobacco products in the past 12 months and complete this program, will have the tobacco surcharge removed from future premium payments for this Plan Year.
