Home-based Department

DEPARTMENT STAFF:

DEPARTMENT INFORMATION:

Home-based is the most restrictive placement on the continuum of special education services. The Home-based Department provides services for students for whom the IEP Team determines instruction should be provided in the home, at a public library, or other non-school based facility. Typically, home-based instruction is provided for students who have been found in violation of school rules at a disciplinary panel and have been separated from the local school as a consequence. A county coordinator (cluster, program or special education coordinator) must attend all IEP meetings in which home-based services will be considered for any reasons other than a disciplinary panel.

DEPARTMENT PROCEDURES

  The Home-based Department receives paperwork on the student who is separated from his/her local school or school of record.

 

 The Facilitator secures a teacher based on the student’s needs and the student’s home location.  

 

  The parent is contacted by the Facilitator to verify/confirm the address and the telephone number of the student who is to receive services.

 

  A Notification for Home-based instruction is sent to the Home-based teacher and the appropriate school personnel at the school of record.  This notification includes the courses and the number of hours per week the student will need as determined by the IEP team.   

 

  The Home-based teacher contacts the case manager at the school of record to obtain assignments and contacts the parent to schedule instructional sessions based on the recommendation of the IEP team.

 

DELIVERY OF HOMEBASED SERVICES

                      The delivery models are:

·         Direct Instruction

·         Tele-class

·         Combination (direct/ Tele-class)

The number of hours a student receives per week is determined by the IEP team.
