Registration Steps for the 2012-2013 School Year This is only for students who attended White Oak last year |

STEP 1:
Complete this required online registration form for each returning
student. The last day to complete the form will be July 23rd. The
information entered is secure. Required school forms will be generated
from the information given. STEP 2: Starting July 20th, please log into MyPaymentsPlus. (Please set up an account if you did not do so last year.) A. Once on the site, you will need to read and accept the terms and conditions of the Gwinnett County Student Handbook, White Oak Student Handbook, AKS brochure, and Acceptable Use of Electronic Media Policy. (mandatory) B. You will then continue on within the site to purchase school supplies, periodicals (required for all), headphones (new students only), and book-boxes (new students only). You will also have to option to contribute to the clinic and technology departments. These contributions are vital in supporting the needs of the school. To view the list of the supplies your child will receive when you purchase the supplies through White Oak, click here. STEP 3: Please complete this online PTA form. STEP 4: If you would like information about our school nutrition program, please click here. Returning families currently receiving free and reduced lunch or those interested in applying for free and reduced lunch must complete applications annually. This application may be completed online by clicking this link. STEP 5: Bring the following documents into the main office, if applicable: A. If your child's immunization record has expired, you will need an updated Immunization Form from your child's doctor or the Health Department. Click here for for more details. B. Residency Affidavits are only valid for 1 school year. If the Residency Affidavit we have on record has expired, please print this new one, complete it, and have it notarized. Return it to school with required documentation. STEP 6: If you have not created a GCPS Parent Portal account, download and complete the Parent Portal form . Note: you must bring the new form into the main office along with Driver's License for verification. STEP 7: If you are interested in student insurance coverage, learn more about it by reading the documents below: A. English Letter of Explanation, English Student Insurance Application B. Spanish Letter of Explanation, Spanish Student Insurance Application STEP 8: Attend the Open House/Orientation on Thursday August 2nd (TBA.) On this day, you will finalize afternoon transportation for your student, collect a car rider or bus number (if applicable), meet your child's teacher, pay for supplies and contributions on MyPaymentsPlus (if you have not already done so), visit the clinic if needed, join PTA, purchase White Oak Spirit Wear, and more. |
