Blue Star Level

Program Audit - ONE per SCHOOL (via online QUIA Survey only - no other document required) Updated annually
Yearlong Master Calendar - ONE per SCHOOL (targeted activities month by month for nine months - create your own document)
Weekly Calendar - ONE per COUNSELOR ('real-life' snapshot of one week - create your own document)
Identify School Counseling Program Goals/Beliefs (minimum three program goals), one per target area (attendance, behavior, academic or other) based on school goals
Closing the Gap Action Plan or Expanded RBES Goal - ONE per SCHOOL, one target area {attendance, behavior, academic or other} Updated annually
Partnership Agreement - ONE per COUNSELOR (Part-time Graduation Coaches submit TWO - one for counseling portion and one for graduation coach portion of your job responsibilities) Updated annually
End of Year Results Report for CTG Action Plan/Extended RBES- Submit results on original Closing the Gap Action Plan document Updated annually

Red Star Level

All items from the Blue Star Level (Program Audit, Yearlong Master Calendar, Weekly Calendars, 3 Program Goals, 1 Closing the Gap Action Plan or Expanded RBES/End of Year Action Plan Results Report and Partnership Agreements must be current for the school year)
Advisory Council - Identify members; provide agendas and minutes from two meetings (could be a spring meeting from previous year with fall meeting for current year)
Mission Statement, Philosophy Statement
Standards and Competencies Crosswalk Tool
Impact Over Time - Updated annually

Silver Star Level

All items from the Blue and Red Star Levels (Program Audit, Yearlong Master Calendar, Weekly Calendars, 1 Closing the Gap Action Plan or Expanded RBES/End of Year Goal Results Report, Partnership Agreements, 3 Program Goals, Advisory Council Information, Mission, Vision and Philosophy, Standards and Competencies Crosswalk Tool, Impact Over Time)
Classroom Curriculum Action Plan- Minimum three exemplary classroom lessons on one unit/topic (per school) directly linked to the school counseling philosophy and mission and GCPS counseling Standards and Competencies.
Classroom Curriculum Lesson Plan Template - complete three forms - one for each of the three exemplary lesson plans
Classroom Curriculum Results Report - at the bottom of the Classroom Curriculum Action Plan. This report summarizes the three exemplary classroom lessons including process, perception and results data.

Gold Star Level

All items from Blue, Red and Silver Star Levels (Program Audit, Yearlong Master Calendar, Weekly Calendars, 1 Closing the Gap Action Plan or Expanded RBES/End of Year Goal Results Report, Partnership Agreements, 3 Program Goals, Annual End of Year Results Report for 3 Goals, Results Report: Impact Over Time (updated with current year information) Advisory Council, Mission , Vision and Philosophy, Standards and Competencies Crosswalk Tool and Classroom Curriculum Information) (must be current for the School year; not historical)
Small Group Curriculum Action Plan - Minimum ONE per SCHOOL, (at least four sessions same topic); linked to standards and competencies and school counseling program goals; process, perception and results data
Small Group Lesson Plan – complete four forms - one for each small group session activity
Small Group Curriculum Results Report – at the bottom of Small Group Curriculum Action Plan - Reflects standards and competencies and is linked to school program goals.
