Students new to the area may register at Patrick beginning Tuesday, July 10th through August 2nd between 9am and 2pm on Tuesday and Thursday only.
The following documents are needed to register:
1. Birth Certificate-Original or Certified Copy
2. Immunization Certificate (State of Georgia Form #3231)
3. Vision, Hearing and Dental Certificate (State Form #3300)
4. Social Security Number (optional)
5. Two proofs of Residency
Examples: Deed, Lease or Contract for residence
Utility Statement or Connection letter showing your name and address
Kindergarten Registration
Documents you will need to register a Kindergarten student:
1. Original or certified Birth Certificate and/or passport
2. Immunization Certificate (State of Georgia Form #3231)
3. Vison, Hearing and Dental Certificate (State Form #3300)
4. Two proofs of Residency:
a. Two current utility bills. ( We will only accept water, gas, or electric from the last 30 days. No phone bills.)
b. A rental agreement or settlement statement of the purchase of the house.
5. Social Security Card (optional)
a. Parents may choose not to provide this information by initialing the social security section of the enrollment form.
6. Picture ID of parent that matches Birth Certificate.
