Downloadable versions of state-required home study forms are available in Adobe Acrobat PDF format. Please follow these instructions.
Step 1:
You will need Adobe Acrobat Reader installed on your machine. If you do not have the reader, click the button 'Get Acrobat Reader' and follow the instructions provided at that web site for downloading and installation.
Step 2:
Determine which file you wish to download, and save the file according to how your browser works.
Step 3:
After downloading the file, open it using Adobe Acrobat Reader.
The forms allow you to fill out the required fields on the screen before printing.
Your changes will not be saved in the file. Please keep a printed copy of all completed forms for your records.
Click the Email Report button at the bottom of the Monthly Attendance Report to submit the completed form electronically.
