State Health Benefit Plan Dependent Audit

Gwinnett County Public Schools has been notified that State Health Benefit Plan (SHBP) is conducting a Dependent Eligibility Verification Audit. The intent of this audit is to ensure that only eligible dependents are covered under SHBP.

An email notification as well as a letter will be sent via U.S. mail to each SHBP member who has at least one enrolled dependent. The letter will detail the steps and documentation required to verify the eligibility for enrolled dependents. SHBP members must complete and sign the letter and submit it, along with the required documentation, directly to SHBP by the deadline indicated on the letter. Documents may be submitted by fax, mail, or uploaded to a secure website.

If you receive the letter and have questions regarding the audit, please contact SHBP at 1-855-838-6938. Members that receive correspondence from SHBP should read it carefully as there are specific due dates as to when documentation needs to be submitted.
